I am the legal guardian of (singer) and do/do not give consent to the Edmonton Young Voices organization to use the name/image of (singer). Edmonton Young Voices and its agents have permission to utilize my child's photographic image in Concert programs, in its public relations and communication materials for a period of five years from the date this consent is signed. I realize that I may withdraw my consent in writing at any time by contacting the organization.

I understand that the photograph(s) may be used in a publication, print advertisement, direct-mail piece, electronic media (e.g. various picture & video digital files on the internet including our website and other marketing channels, etc.) or other communication, but children will never be identified by name.

In giving my consent, I hereby release and hold harmless the Edmonton Young Voices and their agents from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever. I hereby relinquish any and all personal or proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any photograph of my child be used.
